Responsible for managing, organizing, and safeguarding patient medical records in healthcare facilities such as hospitals, clinics, and medical centers. They ensure that all patient information is accurately documented, stored securely, and accessible when required by healthcare providers
Maintain Patient Records: Organize and manage paper and
electronic patient medical records, ensuring that all information is complete,
accurate, and up to date.
Data Entry and Update: Input patient details and
treatment information into the hospital or clinic's electronic health records
(EHR) system.
Confidentiality and Security: Ensure that patient data is stored
securely and confidentiality is maintained in compliance with privacy laws such
as HIPAA (Health Insurance Portability and Accountability Act).
Retrieve Records: Access and retrieve patient
medical records as requested by authorized healthcare providers in a timely and
efficient manner.
Record Audits: Perform regular audits of medical
records to ensure accuracy and completeness, making corrections as needed.
Compliance and Documentation: Ensure all records meet regulatory
standards and policies related to healthcare documentation and patient privacy.
Coordinate with Healthcare Providers: Collaborate with doctors, nurses,
and other healthcare professionals to ensure that medical records are current
and accessible.
File Management: Oversee the filing system for
paper-based records and ensure that electronic systems are properly managed.
Patient Information Requests: Handle requests for patient medical information, ensuring that they are processed in accordance with legal and ethical guidelines.
